1. Information you provide

Contact/request forms may collect your name, email, phone number, request topic and message. The site does not ask for SSNs, payment-card numbers or medical records. Please do not include sensitive health information in a general form.

2. Technical information

Servers receive IP address, request time, browser/user-agent and the referring page. Essential PHP session cookies support form security. Cart selections, if applicable, are stored locally in the visitor’s browser rather than on the server.

3. Messaging consent records

When a visitor submits a form, we record the Account Notifications SMS checkbox state separately from Terms/Privacy acceptance, plus the exact disclosure version, UTC timestamp, IP, user-agent and referring page. SMS is optional and limited to transactional/account-service messages; no marketing SMS consent is requested by this form.

4. Why we use data

We use request data to respond to inquiries, operate and secure the site, maintain communication choices, honor opt-outs, investigate abuse and meet legitimate legal obligations. We do not use contact details to issue a prescription or an insurance policy.

5. Sharing and service providers

We may use hosting, security, messaging and email vendors to operate the website or respond to a request. Mobile numbers and SMS opt-in information are not sold or shared with unrelated third parties for their own marketing purposes. We do not sell consumer contact lists.

6. External links

Following an outside link takes you to another company’s website, which has its own policies. We do not send medical histories or form answers to independent telehealth links from this website.

7. Retention and security

Website requests and consent logs may be retained for operational, legal and consent-audit purposes. We use reasonable safeguards but no system is absolutely secure. The developer must keep private logs outside the public webroot.

8. Your choices

You may decline optional SMS; reply STOP to revoke SMS consent; use the online Opt Out page; or contact customer service to seek access, correction or deletion of information, subject to applicable laws and verification requirements.

10. Minors

The website is intended for adults 18 or older and does not knowingly seek registrations from children.
